1. I am the Claimant in this matter and make this statement from facts within my own knowledge except where otherwise stated.

2. On 15th January 2025, I entered into a written contract with the Defendant for the supply of professional consulting services.

3. The contract was signed at the Defendant's offices at 123 Business Park, London, and I have kept a copy of the signed agreement.

4. The Defendant agreed to provide monthly consulting services for a period of 12 months, with payment due within 30 days of each invoice.

5. I performed my obligations under the contract and submitted invoices on the last day of each month as agreed.

6. Despite multiple reminders, the Defendant has failed to pay invoices totalling £12,500 which are now more than 90 days overdue.

7. I have attempted to resolve this matter amicably by sending three reminder letters and making several phone calls to the Defendant's accounts department.

8. The matters set out in this witness statement are true to the best of my knowledge and belief.
